| Combination / Situation | Why It Matters |
| Opioids + alcohol, benzos, or sedatives | Greatly increases risk of profound sedation, aspiration, and fatal respiratory depression. |
| Opioids + xylazine | Naloxone reverses the opioid component, not xylazine. Continue airway and breathing support. |
| Stimulants + opioids | One drug can mask the subjective effects of the other without eliminating overdose risk (respiratory failure, stroke, overheating still occur). |
| MDMA / Serotonergic drugs + antidepressants | Risk of serotonin toxicity (agitation, confusion, sweating, high fever, muscle rigidity, seizures). |
| Counterfeit pills or powders | Contents may not match expectations; lookalike prescription pills frequently contain illicit fentanyl or nitazenes. |
